403(b)(9) Retirement Contributions

Church Benefits Board (CBB) partners with Empower as our record keeper to manage retirement accounts.

New Employer Application

This form should be completed by new employers (not currently connected with Church Benefits Board) that want to submit 403(b)(9) contributions for employees.

Adding New Employees to Empower

This form is a resource for current employers who have access to the Empower Plan Service Center. Employers should have new employees complete this form then use the information to add an employee to the church’s account to submit 403(b)(9) contributions. This does not need to be returned to Church Benefits Board.

Sample Salary Reduction Agreement
The following is a sample Salary Reduction Agreement (SRA) for our churches and employers to use. The SRA does not need to be sent to CBB, but rather is important for the employer to have on file in the event an employee wishes to contribute to their retirement plan with Employee Voluntary Deferrals. Any changes to an Employee Voluntary Deferral would require an updated SRA to be completed by the Employee and Employer.

Church Benefits Board (CBB) partners with The Hartford to provide this employer elective benefit of Life, Long-term Disability, and Accidental Death & Dismemberment to active employees.

New Enrollment Form

This form should be completed by participants & employers who are seeking to enroll in the Church Benefits Board (CBB) Life, LTD & ADD Insurance. If you are a new employer who does not already have an account with CBB, please be sure to indicate that information on the form where applicable.

Compensation Change or Cancel Benefit Form
This form should be completed by current employers who need to:

  • Send updated compensation information to adjust monthly premiums for participants
  • Cancel a benefit for employee(s) due to termination or separation from service
