Church Benefits Board Financial Relief Grant Application

The purpose of this secure form is to allow CBF Church Benefits Board to collect relevant information related to financial relief grants for ministers who serve congregations within the Cooperative Baptist Fellowship. Funds for such grants are available from generous gifts from private donors and foundations. Grantees may only receive up to $5,000 in financial relief grants within their lifetime. I understand that financial relief grants are intended for legitimate financial needs for ministers who serve congregations, institutions, and/or organizations who partner with or support the Cooperative Baptist Fellowship. As needed, I will present additional documentation to prove this need. I am also aware that financial relief grants are limited to up to $10,000 per individual per lifetime. I understand that relief grants are made possible by generous gifts from private donors and foundations. I submit this request to the financial relief grant review committee.
